Your new patient portal has arrived. OnPatient is no longer servicing your patient account and we listened to your feedback searching for an easier option for all involved, your doctor, yourselves, and our teams. These are your one-time patient portal set-up instructions.


Follow these steps to get setup:


  1. Print these instructions.
  2. Click Here: Patient Portal Setup Page
  3. Go to your email provider (gmail, yahoo, msn, etc).
  4. You will see an email from "OptiMantra Patient Portal" with the subject "Reset Password"
  5. Click the link that is provided in that email.
  6. You will now see "OptiMantra Reset Password Patient Portal"
  7. Enter the same email address
  8. Type your preferred password in the next two boxes.
  9. Click Reset My Password
  10. If done correctly, it will automatically direct you to the Patient Portal Login.
  11. Enter your email
  12. Enter the password you just selected.


Anytime you need to login to the patient portal moving forward, visit: completehumanco.com and click Login in the top right.


A few things to note:

  1. Upon first login, you will see "Demographic Details, Past Clinical History, CO_Combined Consents..."
  2. It says "Not Completed" because you have not done it in THIS specific system, however, we have everything on file for you already, if you would like to use this opportunity to UPDATE anything, you may do so, otherwise your information is secure on your doctor's side.
  3. Your current medications will port over in your next appointment.
  4. All of your other options are on the left side, if you believe you should see something and you don't, simply email us at support@completehumanco.com.
